OGR Family Contact Program Privacy Policy
1. The OGR Family Contact Program is intended to help Golden Rule Funeral Homes (members of Order of the Golden Rule) improve their levels of customer service. Survey data are not shared with any entity other than the funeral home with which the respondent had contact (“local member funeral home”).
2. Original copies of the surveys are returned to the local member funeral home that the review is about. OGR retains copies of survey responses for as long as OGR deems necessary.
3. Online and scanned paper responses are stored on OGR’s secure internal server for as long as OGR deems necessary.
4. OGR does not record names or contact information for anyone who submits a Family Contact survey unless it is voluntarily given on the survey or in supplemental documents. However, the local funeral home may add a code or tracking number to the survey that will enable it to identify respondents.
5. OGR encourages contact between local member funeral homes and families only when additional information is requested and respondents willingly provide their contact information.
6. OGR does not collect any other data besides the responses submitted by those who visit the secure online survey portal. This data is password protected and can only be accessed by authorized individuals.
7. OGR and the funeral home which the survey is about may publish comments submitted on Family Contact surveys online or in print with or without identifying information such as the names of the respondent and decedent. Comments may appear on OGR and/or the funeral home’s websites, social media sites and printed materials.
